SUGGERIMENTI DI PRATICA CLINICA E DI APPLICAZIONE DELLE LINEE GUIDA
This systematic review updates current literature on the epidemiology and treatment of hypertension in dialysis patients. The association between blood pressure (BP) and cardiovascular (CV) risk is linear in the general population while it is of complex interpretation in dialysis patients, a tenet further again confirmed in 4 studies published in 2010 and 2011. No recommendation can be made for BP specific targets in this population. Because left ventricular mass (LVM) is a powerful indicator of myocardial integrity and functioning, a BP target <140/90 mmHg (the standard target for the general population) seems reasonable for patients with normal LVM or with slight left ventricular hypertrophy. In high risk patients and in patients with LV disorders, the BP target needs to be personalized in a context considering blood volume optimization and interventions aimed at improving LV disorders.
